I confirm I am happy to pay my membership monthly via Continuous card payment(unless otherwise agreed).

I understand that my first payment will commence from three days from my seven day trial end date (unless otherwise agreed) and on that date each month there after.

I can confirm that I have no pre-diagnosed medical conditions that may inhibit my ability during physical exercise.

I confirm I have no medical conditions that may cause harm to other students.                                 

Responsible training                 

I confirm I release Jackson's Boa BJJ, the instructors and fellow students (as a group and as individuals) for liability of any injury, damage or illness sustained by me while participating in all training, including seminars.

I confirm I am happy to be governed by – and understand the consequences of – the academy rules(of which I have read). 

Image use

I agree that my image can be used on the clubs social media platforms for promotional purposes.

Cancelling a private class inc block bookings

I understand that if I fail to show for a private class or cancel within 24 hours of the date of the private I am still liable to pay for the private session

I understand that any block bookings are none refundable under any circumstance.
